If you are new to my practice, please fill out the Indie Initial Intake and Consent Forms online before you come for your visit. **IF YOU ARE FEELING SICK (Sneezing,Coughing, Nasal Congestion, Sore throat etc) PLEASE CALL TO RESCHEDULE.** I wear a mask during your visit and have extensive safety and cleaning protocols in place to protect you during your visit. If you would like to know more about my Covid protocols, please feel free to contact me. Please plan to arrive about 5 minutes before your scheduled appointment.

My practice is located at 215 Western Ave, Suite C, Petaluma CA 94952. **THE DOOR CODE IS 373372** to enter the building. Please make a note of it. The nearest cross street is Liberty. The office is up above Alphabet Soup. The office is upstairs and the last office on the right. There is free two hour parking on Western Ave. and surrounding streets. Although spots are generally easy to find, please allow extra time to find parking. Cancellation policy I request 24 hours notice if you cannot make it to your appointment. Any appointments cancelled with less than 24 hours notice will be billed the full appointment fee.
